IT-MID Makeup Examination Application Form for the Spring Semester (2) 2023-2024

]Dear IT Student, If you have missed the MID exam due to health issues / death in family / accident / other reason, please fill out the form and upload a supporting document with a ministry / ROP seal. The evidence is very much required for scheduling the Makeup exam. The examination committee will conduct the makeup exam for the eligible students. You should produce the original supporting document at the time of makeup exam. Note: For the Makeup exam, an appeal/excuse should be submitted within 3 days from the date of the exam. If the appeal/excuse is submitted after the due date then it will not be considered for the makeup exam as per rules.
