Applied Sciences: Mid-Semester Make-up Exam Application Form

Dear Students, If you missed the Mid-Semester exam due to health issues or other problems, please fill out the form below and upload the sick leave certificate or any valid proof. In addition, you should submit the original evidence to the examination committee (M-119) within three days from the date of the missed exam. - Fill out the form using your ID login, NOT your friends' or your advisors'. - The application will NOT be accepted if: Sick leave from a private clinic/hospital without authentication from governmental health centers. An attendance Certificate from the health center or private clinic/hospital. The application form is submitted after three days from the exam date.
